H10: additional information was received, and the file was reassessed for reportability and determined to be reportable as malfunction.H10: manufacturing review: a manufacturing review was not required as this is the only complaint reported to date for this product and lot.Investigation summary: one ultraverse 035 dilatation catheter was returned for evaluation.Multiple kinks were noted to the balloon. No rupture was observed to the balloon. An in-house presto inflation device was used to inflate the balloon and water was noted streaming from the proximal end of the guidewire luer and at the distal end of the catheter shaft. No other functional testing performed. Therefore, the investigation is unconfirmed for the reported balloon rupture as no rupture was observed to the balloon, but the investigation is confirmed for the identified leak as water streamed from both the proximal end of the guidewire luer and the distal end of the catheter shaft. The investigation is also confirmed for the identified material deformation as multiples kinks were noted to the balloon.A definitive root cause for the reported balloon rupture, identified leak and material deformation could not be determined based upon the provided information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.H10: d4 (expiry date: 08/2024), g3, h2, h6 (device).H11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
