Event or Problem Description
Device report from synthes reports an event in japan as follows: it was reported that on (b)(6) 2024, the patient underwent a removal of ptp large implants. The locking screw in question could not be removed by a screwdriver, and an extraction screw was used for its removal.Naturally, the screw was not unlocked, and the extraction screw broke off.The sales rep did not inform the surgeon of the correct procedure. After that, the surgeon removed the plate by a carbide drill. When the hollow reamer was attached to the power tool and rotated, rotation was eccentric. The rotation was eccentric no matter how many times the surgeon engaged the hollow reamer with a jacobs chuck. The surgeon did not use that hollow reamer but used a one-size larger reamer to remove the screw, and the screw was removed without any problem. The surgery was completed successfully within 30 minutes delay.Patient status/ outcome: stable the surgeon commented that the axis wasn't straight.Although not apparent, rotating hollow reamer revealed that the axis was bent. There are no pieces in the patient. The surgeon confirmed by x-ray.No further information is available. This report is for one (1)hollow-reamer-compl anticlockwise cuttin. This is report 3 of 3 for complaint (b)(4).

Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.A manufacturing record evaluation was performed for the finished device product code: 309.450, lot number: 26p3155.It was electronically reviewed and no nonconformances / manufacturing irregularities were identified during the manufacturing process. The product was released on: 11-mar-2020, manufacturing site: jabil bettlach. The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the sparereamertube f/309.450, (309.480), was returned in an assembled condition with hollow-reamer-compl anticlockwise cuttin/(309.450) and the sparecenterpin f/309.450 (309.470).No cosmetic defects were observed on the surface of the device.A dimensional inspection was not performed as it is not applicable to the complaint condition.A functional test was performed to attempt to disassemble the returned devices. The process was successful the devices work as intended. The complaint condition was not replicated. The screw extraction set "instruments for removing synthes screws" handling technique was reviewed.Following relevant statements were found. The hollow reamer (complete) consists of three individual parts: reamer tube, centring pin and shaft.1.Connect the centring pin and the shaft (left-hand thread).2.Screw the reamer tube over the centring pin (left-hand thread).Note: there is no thread connection between the space centring pin and the shaft for hollow reamers. In these models, the space centring pin is positioned loosely in the hollow reamer. The reamer tube and the shaft are then screwed together (left-hand thread). The overall complaint was unconfirmed as the observed condition of the hollow-reamer-compl anticlockwise cuttin would not contribute to the complained device issue. There is no indication that a design or manufacturing issue has caused the complaint condition and hence the root cause cannot be determined.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications. If information is obtained that was not available for this medwatch, a follow-up medwatch will be filed as appropriate.
