Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: according to the information received, ¿it was reported that the surgeon noticed scratches and a foreign substance on the surface of a sz 8 ps left femur. It was unknown if there was a surgical delay¿.The product returned to medtech orthopaedics for evaluation. The complaint unit was forwarded to medtech orthopaedics raynham for a manufacturing investigation.Visual analysis of the returned attune ps fem lt sz 8 cem confirms the presence of two uniformly parallel scratched lines across the articulating polished surface. In addition, a clear/white foreign substance was observed adhered to the polished surface in multiple locations was observed.Since the parallel scratches passes through one of the hardened deposits of foreign material (clear / white substance) and removed the foreign material in its pathway; it can be concluded that the scratches occurred after the foreign material contacted the implant.Based on the manufacturing investigation, it is not expected that observed cosmetic defect originated during medtech orthopaedics raynham manufacturing.A manufacturing record evaluation was performed for the finished device [150410108 / m3452u] number, and no non-conformances / manufacturing irregularities were identified during manufacturing. The overall complaint was confirmed as the observed condition of the attune ps fem lt sz 8 cem would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation was performed for the finished device [150410108 / m3452u] number, and no non-conformances / manufacturing irregularities were identified during manufacturing.Device history review: a manufacturing record evaluation was performed for the finished device [150410108 / m3452u] number, and no non-conformances / manufacturing irregularities were identified during manufacturing.
