Additional Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue. It has been over 8 years since the subject device was manufactured. The device was returned to olympus for inspection, and the customer's allegation was confirmed.Based on the results of the investigation for the events of moisture found, internal lens burned and foreign debris in optical system, the moisture inside the instrument can be traced back to a leak in the instrument due to its age, frequent reprocessing and use.The liquid from the leaking drying ring settles on the flat glass and leaves drying stains. In this case, the stains have not dried completely as the moisture cannot escape and have left yellowish stains.Olympus will continue to monitor field performance for this device.
