Event or Problem Description
It was reported that one day post implant the left ventricular (lv) lead was nonfunctioning with no capture. The lv lead was reprogrammed subthreshold. It was noted that the lv lead was inserted in the right ventricular (rv) lead port of the implanted device, this is considered off label use.Due to the anatomical location of the lead and the lead being inserted in the rv port the the device feature that automatically monitors pacing thresholds at periodic intervals was not running. The lv lead remains in use. no patient complications have been reported as a result of this event.
