On february 28, 2024, mentor was notified that the right breast implant was found to be intact during removal surgery. However, it was reported that the patient was diagnosed with bilateral baker grade ii-iii capsular contracture of the breasts.Rupture is no longer applicable to this file.As an event for the contralateral side was indicated, another file was generated to document the event. This report is for the right breast prosthesis.Refer to manufacturing report number 1645337-2024-03566 for the contralateral event. On march 05, 2024, mentor was notified that the right implant was damage by the surgeon during the surgery. On march 19, 2024, the mentor analysis lab received the suspect medical device for evaluation. On march 21, 2024, mentor completed an evaluation on the returned device.Mentor conducted a visual inspection and microscopic examination of the device.Visual analysis of the returned sample revealed that the breast implant had an area of siltex cracking on the posterior view.In addition, a tear was noted within the siltex cracking measuring approximately 1.1 cm.A microscopic examination was performed and instrument damage was not found on the area of the tear. The evaluation determined that the possible cause of the rupture was consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses.Mentor concluded that the capsular contracture in the patient´s breast was the result of the body´s individual physiological response to the implantation of a foreign object in soft tissue.Capsular contracture is a known complication associated with these devices and is referenced in our current product insert data sheet. The american society of plastic surgeons recommends and encourages member surgeons to always submit breast implants, capsules, and effusion to pathology for examination.Manufacturer¿s reference number: (b)(4).
