It was reported that a patient underwent a primary breast augmentation surgery with implantation of a 470cc mentor memorygel breast implant prosthesis.Post-operatively, the patient was diagnosed with bilateral implant displacement and breast ptosis by a medical professional. As a result, the patient underwent bilateral removal and replacement with 650cc mentor smooth round moderate plus profile saline breast implants on (b)(6) 2024.However, during the surgery, a ruptured left breast implant was discovered. There were no reported procedural delays or patient consequences due to the discovery. This report is for the left breast prosthesis.Refer to manufacturing report number 1645337-2024-02400 for the contralateral event.

On february 28, 2024, the mentor analysis lab received the suspect medical device for evaluation. On march 06, 2024, mentor completed an evaluation on the returned device.Mentor conducted a visual inspection and microscopic examination of the device.Visual analysis of the returned sample revealed that the breast implant had an area of shell abrasion on the posterior view. in addition, a tear was noted within the shell abrasion measuring approximately 7.6 cm.A microscopic examination was performed and instrument damage was not found on the area of the tear. The evaluation determined that the possible cause of the rupture was consistent with normal wear.Shell abrasion suggests in-vivo folding or creasing of the device. This may be the result of the following factors: continuous and sustained stresses to the device such as too small breast pocket and folding or wrinkling of the shell in the breast pocket. In some cases, the breast implants may also wear out over time. implant displacement/migration may occur from improper implant sizing and/or placement, i.E., when the implant is too large or the pocket too small or when there has been inadequate preoperative assessment of stresses causing movement of the prosthesis.Implant displacement/migration is a known complication associated with these devices and is referenced in our current product insert data sheet. Some breast ptosis is a normal component of the mature breast.Ptosis becomes undesirable when the breast parenchyma predominates below the areola, droops considerably below the inframammary fold and the nipple points downward.Augmentation alone, without consideration of the ptosis, can produce a less than desirable cosmetic result known as a "rock in sock" deformity or an increased ptotic appearance.Ptosis is a known complication associated with these devices and is referenced in our current product insert data sheet.Manufacturer¿s reference number: (b)(4).
