Product complaint # (b)(4).Investigation summary: according to the information received, hcp is reporting the setting instrument for the medullary canal stopper broke off when it was unscrewed. No force was used. The approximately 6 cm long fragment could not be removed and had to remain in the patient. The product was not returned to depuy synthes, however photos were provided for review.See attachment (b)(4) photos received from customer. The photo investigation revealed that the hardinge fem cement rester was found broken from the shaft, the fracture originated from the number 7 marked on the shaft, indicating that approximately 6 cm from the tip broke off the instrument. No postoperative x-rays were provided to confirm the foreign body left in patient. The shaft breakage is consistent with the user applying excessive leverage/torque on the hardinge cem rester int in a side-to-side or circular pattern.Since the device was not returned, a dimensional inspection cannot be performed. The overall complaint was confirmed as the observed condition of the hardinge fem cement rester would contribute to the complained device issue.Based on the investigation findings, potential cause can be attributed to unintended use error and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing records evaluation (mre) was not performed as no lot number was provided for this device.

Product complaint # (b)(4).Investigation summary : hcp is reporting the setting instrument for the medullary canal stopper broke off when it was unscrewed. No force was used. The approximately 6 cm long fragment could not be removed and had to remain in the patient. The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual inspection of the returned sample revealed that the hardinge cem restr int was found broken from the shaft, the fracture originated from the number 7 marked on the shaft, indicating that approximately 6 cm from the tip broke off the instrument. No postoperative x-rays were provided to confirm the foreign body left in patient. The shaft breakage is consistent with the user applying excessive leverage/torque on the hardinge cem restr int in a side-to-side or circular pattern.A dimensional inspection was unable to be performed due to post manufacturing damage.A functional test was unable to be performed due to post manufacturing damage. The overall complaint was confirmed as the observed condition of the hardinge cem restr int would contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to unintended use error and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : manufacturing records evaluation (mre) was not performed as no lot number was able to be retrieved for this device.
