Product complaint # (b)(4).Investigation summary it was reported that surgeon was tensioning bal sizer with lamina spreader.Upon further inspection after the case, sales rep noticed small cracks on both sides of the paddles causing the sizer to be flexed and needs replaced. There was no delay in the surgery and no pieces broke off within the patient. The product was returned to j&j medtech orthopaedics for evaluation. The j&j medtech orthopaedics team conducted a visual inspection of the returned device along with an investigation performed by supplier.Visual analysis of the returned device revealed that the mto attune balancer sizer had cracks on the weld that joins the outer slide tube and the outer slide tube collar. As per the reported event, a lamina spreader was used.Repeated use between the upper and lower foot of the device concentrates the force applied on the weldment joining the outer slide tube and the outer slide tube collar. This continued use of the lamina spreader in this manner can result in the development of cracks in the weldment.Continued application of force through the lamina spreader may propagate these cracks, ultimately leading to a progressive weld failure. The lifecycle requirements of the device are event related and depend on the use and inspection of the device in clinical practice. As the device can be damaged on the first or 100th use, the device must be properly inspected prior to each surgical use.Refer to the device/country specific ifu for information related to end of life, reprocessing instructions, and inspection procedures. All welding was performed to specifications and weld analysis indicated that the weld met the requirement for penetration and size.A functional test was not performed since it was not applicable to the complaint condition.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the mto attune balancer sizer would have contributed to the complained issue.Based on the investigation findings, the potential cause is traced to end of life, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.Corrected: d4 (primary udi #), h3.
