Medtronic received a report that the pipeline failed to open. The patient was undergoing surgery for treatment of a saccular, unruptured aneurysm in the right internal carotid artery ophthalmic artery with a max diameter of 5.63 mm and a 4.64 mm neck diameter.Landing zone: distal: 3.57 mm and proximal: 4.64 mm.It was noted the patient's vessel tortuosity was moderate.Dual antiplatelet treatment was administered. The angiographic result post procedure was normal. It was reported that after the ped2 stent was delivered into place via the microcatheter phenom27, stent deployment began. After the tip of the pipeline stent was deployed, the tip failed to open smoothly. At this time, the middle catheter was raised to provide stronger support. The stent was recovered and the tip was deployed again. After multiple attempts of pushing and pulling, the problem still could not be solved, and the stent was subsequently withdrawn from the body. The ped2-475-20 stent was replaced and the surgery was successfully completed. The pipeline was not used for an indication that is off-label.The reported device and any accessory devices were prepared as indicated in the instructions for use (ifu). No patient symptoms or further complications were reported as a result of this event.

Additional information was received that the cause of the failure to open was not determined. It was noted that the tip end of the pipeline was in the straight section of the middle cerebral artery, and after replacing with the new stent, the same operation could be opened smoothly.Additional step/device attempted to open the pipeline included pushing intermediate catheter to go upper.

Product analysis: as found condition: the pipeline flex shield embolization device was returned for analysis within shipping box; and within a plastic bio-pouch.Damage location details: no bend was observed on the push wire. The hypotube was found to be stretched. The shrink tubing was found intact but pulled back from the proximal bumper. The distal and proximal dps restraints were found to be intact. The dps sleeves were found intact with no signs of damage. No defects were found with the proximal bumper, re-sheathing pad, re-sheathing marker, distal marker, and tip coil. The braid was returned already detached from the pusher; therefore, the proximal and distal ends could not be identified. The distal and proximal ends of pipeline flex shield braid were found fully opened and damaged no other anomalies were observed.Testing/analysis: n/a conclusion: based on the analysis findings, the pipeline flex shield could not be confirmed to have failure/incomplete open distal (flex) as the device was resheated. In addition, the proximal and distal ends could not be identified.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Medtronic will submit a supplemental report if additional relevant information becomes known.
