Event or Problem Description
It was reported that the procedure was to treat the first diagonal branch artery lesion.A 2.25x23 xience skypoint stent delivery system (sds) was attempted to be placed via radial approach but was unsuccessful.During removal the sds got caught on a previous implanted stent and the stent was stripped off the delivery system. There was an attempt to snare the device out but was unsuccessful.A semi complaint 1.5x15mm balloon was used to pull the stent back to the proximal artery and the stent was crushed against the artery wall.A 3.0x23mm xience skypoint was then deployed across it.There were no adverse patient sequela and no clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents from this lot.The investigation was unable to determine a conclusive cause for the reported failure to advance. The reported difficulty to remove, stent dislodgement and treatment appear to be related to circumstances of the procedure. There is no indication of a product quality issue with respect to manufacture, design or labeling.Na.
