Pma/510(k) #: k172057.Device evaluation: the device evaluation could not be completed as the device or photographic evidence of the device was not returned for evaluation.Manufacturing records: prior to distribution, all jpws-10-14 devices are subjected to functional checks and visual inspection to ensure device integrity.A review of the manufacturing records for jpws-10-14 device of lot number c2115908 did not reveal any discrepancies that could have contributed to this complaint issue.Review historical data: a review of the manufacturing records for the jpws-10-14 device confirms the failure mode has not previously occurred with the current lot number.Based on the information available to date, there is no evidence to suggest that there are any manufacturing issues associated with lot number c2115908.Instructions for use and label: the notes section of the instructions for use (ifu0098), which accompanies this device instructs the user to inspect the device prior to use : "visually inspect with particular attention to kinks, bends and breaks. If an abnormality is detected that would prohibit proper working condition, do not use".The user is also advised as per step.2 of the ifu, "*note: endoscope elevator must be open to allow the stent to exit." there is no evidence to suggest that the customer did not follow the instructions for use.(ifu0098).Image review: an image was not returned for evaluation.Additional information was requested on the meaning of the phrase ¿plicature du kt porteur¿ from the description of event.Reply received: - please clarify what is meant by ¿plicature du kt porteur¿ kink (ref.Attached: (b)(4)).Root cause analysis: a definitive root cause could not be determined from the available information.A possible root cause could be attributed to the endoscope used, where the lining of the endoscope was not smooth, causing the kink and that this could have contributed to the stent becoming stuck in the working channel of the scope.Another possible root cause is that the endoscope elevator was not fully open which contributed to the difficult advancement of the stent. It may be noted that engineering input states that it is possible the reported events were caused by the elevator scope not being fully open whilst advancing the stent through it which would be user error as per the ifu, even though confirmation of this information was unavailable.(ref.Attached: (b)(4)).Confirmation of complaint: complaint is confirmed based on customer and/or rep testimony.Corrective action/correction: complaints of this nature will continue to be monitored for potential emerging trends.Summary of investigation: according to the customer, impossible to get the plastic stent out of the duoscope.Confirmed quantity of 1 device, confirmed prior to use.According to the initial report, the patient did not experience any adverse effects due to this occurrence. As per medical advisors input ¿ the patient would require stents regardless since the cirl stent was not the consequence of placing two metal stents.¿ investigation findings conclude that a definitive root cause could not be determined from the available information.A possible root cause could be attributed to the endoscope used, where the lining of the endoscope was not smooth, causing the kink and that this could have contributed to the stent becoming stuck in the working channel of the scope.Another possible root cause is that the endoscope elevator was not fully open which contributed to the difficult advancement of the stent. It may be noted that engineering input states that it is possible the reported events were caused by the elevator scope not being fully open whilst advancing the stent through it which would be user error as per the ifu, even though confirmation of this information was unavailable.
