The reported event could be confirmed based on the inspection performed. The device inspection revealed the following: the identification of the returned drill bit could be confirmed based on the catalog # and lot # marked. The reported event could be confirmed as the instrument is broken at its tip.The breakage surface gives indication of a sudden brittle fracture, resulting most probably from excessive bending and/or torsional load. The area close to the breakage is highly damaged, giving indication that the device was probably already worn out before the reported event.Dimensional and material integrity inspection showed the device to be according to specification.Based on investigation, the root cause was attributed to an user related issue. The failure was most probably caused by excessive bending and/or torsional load.Given the condition of the instrument, it is also probable that hcp failed to properly inspect the drill bit prior to the surgery. As a reminder, the instructions for use clearly state that: "before every operation, ensure that all components and devices to be used during the operation are available in the operation theatre and function correctly with each other.Ensure that drilling and cutting tools are sharp." a review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design related problems were found during the investigation. If more information is provided, the case will be reassessed.
