On may 21, 2024, mentor completed an evaluation on the returned device.Mentor conducted a visual inspection of the device.Visual analysis of the returned sample revealed that the breast implant had areas of siltex cracking on the posterior view.In addition, four (4) tears (a - d) were found within the siltex cracking areas, measuring approximately 0.8 cm, 1.1 cm, 0.4 cm, and 6.0 cm respectively. The evaluation determined that the possible cause of the ruptures (a - d) was consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses.A manufacturing record evaluation (mre) was performed, and no anomalies were found related to this complaint. In addition, the mre verifies that the device was manufactured in accordance with documented specification and procedures.Manufacturer¿s reference number: (b)(4).
