Product complaint # (b)(4).Investigation summary: according to the information received, ¿bearings broke¿ the product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that sp2 im rod 400mm presents an overall worn appearance consistent with normal and constant usage for over 19 years. However no signs of breakage nor anything indicative of a device nonconformance were observed on the device.A functional test performed with the returned mating device revealed that the rod was able to pass through the femoral locating device as intended. The locking mechanisms was able move from locked to unlocked as intended and the locating pivoting body could also move freely. The overall complaint was not confirmed as the observed condition of the sp2 im rod 400mm would not contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (gm0305) provided is not a valid finished goods lot#.H10.
