The reported device was received for evaluation.A visual inspection of the returned device found that it is not in its original packaging. The insertion device was returned with the fractured anchor and suture string in place. The anchor is fractured from the proximal end of the suture window. There is biological debris on the returned items.Based on the condition of the product material found during visual inspection, it was determined the device damage was caused by the application of improper/excessive force.Additional material testing is not required.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found no similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate. The root cause was associated with unintended use of the device.Factors that could have contributed to the reported event include: (1) hitting the device tip against a hard surface.(2) using the device as a lever to enlarge surgical site.(3) mechanical displacement of tissue through applied force. No containment or corrective actions are recommended at this time.

H10: h3, h6: the reported device was received for evaluation.A visual inspection of the returned instrument shows no manufacturing abnormalities. The electrode is detached.Product was out of the original packaging. No packaging returned.A review of the customer provided image shows a piece of the original packaging. The device was plugged into the controller and registered setting. The wand was not able to generate plasma and coagulation due to detached electrode.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found no similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A definitive root cause for the reported issue could not be determined.Factors that could have contributed to the failure include procedural variance, or deviations from established protocols, increasing risks and leading to unintended consequences.Other potential factors include: (1) hitting the device tip against a hard surface.(2) using the device as a lever to enlarge surgical site.(3) mechanical displacement of tissue through applied force.Based on this investigation, there is no evidence to suggest a design, material, or manufacturing issue. Therefore, the need for corrective action is not indicated.
