As reported, the edge of the 3dmax light mesh was noted to be broken. This was reported as an out of box event.Photo provided shows a 3dmax light mesh within its clamshell tray.Due to the mesh being inside the clamshell tray the edges of the mesh are not clearly visible.Photo does not assist in determining root cause. As reported the sample is being returned for evaluation; however, has not been received. At this time, no conclusions can be made.If/when the sample is returned and evaluated a supplemental mdr will be submitted.Review of the manufacturing records was performed and found that the lot was manufactured to specification. To date, this is the only reported complaint for this manufacturing lot of (b)(4) units released for distribution in june 2021.Note: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.H3 other text : not returned.
