Product complaint # (b)(4).Investigation summary : pt had a tibial fracture at time of primary implant. Was touch weight bearing to allow cementless tibia to ingrow and the fracture to heal.Patient presented for revision of tibia for subsidence. The product was not returned to depuy synthes, however photos were provided for review.See attachment [(b)(4)].The x-ray investigation revealed that the attune rp tib base sz 5 por, had indeed subsided or migrated, corroborating the reported allegation. While it's important to acknowledge that the implant itself may not be at fault, the surgical decisions and processes surrounding the implantation procedure likely played a significant role in the observed migration. This occurrence suggests a complex interplay between load-bearing requirements for proper implant fixation and the presence of a tibial fracture.Despite the necessity for load-bearing to facilitate implant stability, the fracture likely compromised the structural integrity of the bone, leading to unintended migration of the device. This underscores the importance of careful consideration of surgical techniques, patient-specific factors such as the presence of fractures, bone density and post-operative management protocols to minimize the risk of implant migration in similar cases.Since the device was not returned, a dimensional inspection cannot be performed. The overall complaint is confirmed as the observed condition of the attune rp tib base sz 5 por would contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to user, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : a manufacturing record evaluation was performed for the finished device 150611005/4131282 number, was manufactured on 16-apr-2023.18 parts were manufactured per specification and all raw materials met specification.Device history review : a manufacturing record evaluation was performed for the finished device 150611005/4131282 number, was manufactured on 16-apr-2023.18 parts were manufactured per specification and all raw materials met specification.
