Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: the (agency name) agency has been informed by (hospital name) hospital of an incident with a pinnacle tha acetabulum cup.The operation with tha at private hospital [hospital name and date removed] shortly after a sense of the knee, click and instability of the hip.Re-operation [date and hospital name removed] shows broken plastic liner. The product was not returned to depuy synthes, however photos were provided for review.See attachment "2024030780_img_7300".Review of the photographic evidence revealed that the rim of the line has fractured. One (1) large and three (3) small broken fragments can be observed on the provided evidence.Additionally, some of the anti-rotation device (ards) tabs appeared to be sheared off.Base on the observed damage of the liner and the reported noise and instability of the hip, it is reasonable to conclude that the liner disassociated from the acetabular cup. With the information provided is not possible to determine a potential cause at this moment. The mode of failure of the device is multi-factorial and consideration must be given to all other potential influences such as surgical process, patient variables (e.G.Activity level and use), anatomical considerations and patient changes over time.Any conclusions from the investigational input provided have to be placed into context with all other relevant factors. It is highly recommended to the patient to consult with their healthcare professional for further assessment. The overall complaint was confirmed as the observed condition of the liner would contribute to the complained device issue.Based on the investigation findings, a potential cause is traced to component failure. No corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the device lot number is unknown, therefore a device history review could not be performed. If the lot/serial number becomes available, the record will be re-assessed.

Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: according to the information received, ¿the (agency name) agency has been informed by (hospital name) hospital of an incident with a pinnacle tha acetabulum cup.The operation with tha at private hospital [hospital name and date removed] shortly after a sense of the knee, click and instability of the hip.Re-operation [date and hospital name removed] shows broken plastic liner.¿ the product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the pinn mar +4 10d 36idx52od has multiple fractures around the rim.One (1) large fragment that contains some of the anti-rotation device (ards) tabs and (3) small broken fragments from the lipped section of the liner were returned for evaluation.Additionally, a slightly deformation was found around the fracture areas and near the lipped section.Based on the observed damage of the liner and the reported noise and instability of the hip, it is most likely that the insert has disassociated from the cup. There is no indication that a design or manufacturing issue has caused or contributed to the reported event.Although the liner failure cannot be traced to design or manufacturing, a definite root cause cannot be established at this moment. The mode of failure of the device is multi-factorial and consideration must be given to all potential influences such as surgical process, patient variables (e.G.Activity level and use), anatomical considerations and patient changes over time.Any conclusions from the investigational input provided have to be placed into context with all other relevant factors. It is highly recommended to the patient to consult with their healthcare professional for further assessment.A dimensional inspection for the liner was unable to be performed due to post manufacturing damage.A manufacturing record evaluation was performed for the finished device [121936152 / 3720980] number, and no non-conformances / manufacturing irregularities were identified during manufacturing. The overall complaint was confirmed as the observed condition of the liner would contribute to the complained device issue.Based on the investigation findings, a potential cause is traced to component failure. No corrective and/or preventative action is proposed. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation was performed for the finished device [121936152 / 3720980] number, and no non-conformances / manufacturing irregularities were identified during manufacturing.
