This report is being sent as follow-up no.1 to update section d9, section h3, and to provide the completed investigation results. This reported event has been reassessed and deemed not reportable based upon the investigation of the actual sample.Based upon the inspection of the returned sample the reported event was reassessed and deemed not reportable.Inspection of the returned sample upon receipt found that the one 6fr ds sheath and dilator were returned for assessment. No visual abnormalities were seen on the sheath nor dilator. The sheath valve was subjected to leak testing without anything inserted in it and with the dilator inserted in it.The valve passed leak testing in both instances. The valve was disassembled and assessed. No damage was seen on the valve. The complaint cannot be confirmed for valve leakage because the sample passed leak testing. The exact root cause of the valve leakage experienced by the user could not be determined. The likely root cause of the leak during the procedure is that the catheter that was inserted into the valve was pressed down on the valve in such a way that the leaflets were held open, allowing some fluid to leak.No failure mode was detected on the returned device.Review of the device history record (dhr) showed there were no issues noted during manufacture of the product that could have contributed to this complaint condition.Currently no action is recommended since this risk evaluation is within the predetermined limits in the design failure mode and effects analysis (dfmea). Therefore, this event is currently deemed a non-reportable event.
