Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary it was reported that intraoperatively the both instruments got jammed and the threads bent.Surgery was finished without any delay and without any adverse patient consequences. The product was returned to depuy synthes for evaluation.Visual inspection of the returned device revealed that the rod for reaming guide holder was found bent from the tip, the device was not jammed with the mating device. No other issues were observed, therefore, the reported condition can be confirmed. The tip bent is consistent with the user applying excessive leverage/torque on the rod for reaming guide holder in a side-to-side or circular pattern.Properly handling and attention to the approved use of the device diminishes the risk of failure.A dimensional inspection was not performed as it is not applicable to the complaint condition.A functional evaluation was unable to be performed due to post manufacturing damage. The overall complaint was confirmed as the observed condition of the rod for reaming guide holder would contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to unintended use error and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot please provide: 1) quantity manufactured: (b)(4).2) date of manufacture: 15/02/2024.3) any anomalies or deviations identified in dhr: no non-conformances were found.4) ifu reference: w90966.The dhr analysis of the batches returned shows an initial conformance of those products with regards to their specification. For those batches, there was no deviation or non-conformance corrected: (primary udi number).
