This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue. It has been over twelve (12) years since the subject device was manufactured. An olympus endoscopy support specialist (ess) was dispatched on 11 march 2024, to observe the user facility¿s reprocessing practices from start to finish and provide a reprocessing in-service training, if necessary, to correct and address any reprocessing deviations.During the in-service the ess covered the guidelines on reprocessing the olympus scope per reprocessing manual. The staff also performed a return demonstration to show they understood the process. The customer also understood that the olympus reprocessing manuals are the validated source of instructions.A definitive root cause was not identified, however based on the results of the investigation, the probable cause of the "when the user performed leakage test, they did not check mb-155-leakage tester or mu-1-maintenance unit prior to connection to the scope" malfunctions would likely be related to difference in recognition on device handling or reprocessing steps between olympus recommendation and the user facility. The event can be prevented by following the instructions for use which state: evis exera ii gif/cf/pcf type 180 series reprocessing manual chapter 5 reprocessing the endoscope (and related reprocessing accessories).Olympus will continue to monitor field performance for this device.
