This follow-up report is being submitted to relay additional information. The following sections were updated.H6: component code: mechanical (g04) - stem. One stem was returned and evaluated.Upon visual inspection, it was noted that the device had fractured at the female taper bottom. There was visible gouging and scratches to the profile of the device, along with scuffing to the face and male taper shaft. The returned hip stem was submitted for fracture analysis. The fracture surface showed smearing and biological debris that obscured the artifacts. However, high magnification imaging revealed striations in multiple areas suggesting that the hip stem fractured due to fatigue.A review of the device history records identified no deviations or anomalies during manufacturing. The reported products were reviewed for compatibility with no issues noted.Medical records/radiographs were provided and reviewed by a health care professional.A review of the available records identified findings of the reported issues: right total hip arthroplasty with long stem from a component which appeared to be fractured at the junction of the femoral neck and proximal femoral stem.A definitive root cause cannot be determined. The event was confirmed via x-rays and returned product if any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
