It was reported that on 07 march 2024, a 19mm regent aortic mechanical heart valve ( serial: (b)(6)) was chosen for implantation. After the valve was sutured, a tear was observed on the valve ring. There was no leaflet damage, and no dislodgment of any pieces of the valve. The valve was removed and a replacement 19mm regent valve (serial: (b)(6)) was implanted successfully. The patient remained on bypass throughout the procedure and there was no unusual intervention due to the extended bypass time. There was no patient consequences or clinically significant delay. The patient remained hemodynamically stable throughout the procedure. The patient was reported to be stable.

Additional Manufacturer Narrative
An event of tear observed in valve cuff after suturing was reported. The sewing cuff was noted to have a tear and contained blood/body fluids. There was a crack noted from the top to the bottom rim.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met all specifications. While the root cause of the reported incident could not be conclusively determined, the noted damage may have been caused by some external force applied to the valve which overstressed the carbon material. There is no indication of a product quality issue with respect to labeling design or manufacturing of the device.
