It was reported that the procedure was to treat a moderately calcified, moderately tortuous first diagonal branch. The 2.25x12mm xience skypoint stent was implanted without issue; however, the patient began to experience chest pain, which was confirmed with positive ekg.Angiography was performed and observed acute thrombus had occurred. The stent was ballooned and thrombectomy was performed on the diagonal lesion. It was noted that the correct stent size was selected; however, may have been not been fully apposed to vessel. There was no adverse patient sequela and no clinically significant delay reported. No additional information was provided.

The device was not returned for evaluation.Production record and corrective and preventive actions (capa) reviews were performed and revealed no indication of a product quality issue.Additionally, a review of the complaint handling database identified no similar complaints from this lot.A conclusive cause for the reported patient-device incompatibility (wall apposition) could not be determined.A conclusive cause for the reported patient effects, and the relationship to the product, if any, cannot be determined; however, the subsequent unexpected medical intervention appears to be related to the operational context of the procedure.Patient-device incompatibility (wall apposition) may be attributed to several factors including, but not limited to, patient anatomical morphology, deployment technique, product size selection, or interference from previously deployed devices. In this case, it is possible that the stent may have been undersized for the vessel, as reported by the physician; however, this cannot be confirmed. The reported patient effects of angina and thrombosis are listed in the xience skypoint, everolimus eluting coronary stent system (eecss), electronic instructions for use as known patient effects of coronary stenting procedures. There is no indication of a product quality issue with respect to manufacture, design, or labeling.
