Product analysis: the device returned for evaluation with a sheath was entangled in the jaws and the catheter was bent.Procedure analysis shows thermal cycle was not ran, which aligns with the complaint. An attempt was made to close the jaws which were likely prevented by the sheath entanglement. It was not possible to remove the sheath from the catheter tip without damaging the catheter tip.In an attempt to analyze without further bending to the shaft the tip and sheath were cut off the returned device. The closing gap value read 408 on a power controller showing the shaft was too bent to fully close. The handle was opened, and no issue was found with the soldering.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
