(b)(4).The complaint of misfire/jam - staples not releasing was confirmed based upon the sample received.Visual examination revealed that the first staple appeared to be misaligned. It was observed that the first misaligned staple was positioned upside-down in the cover block.Upon functional inspection, the misalignment of the staples prevented the remaining staples from consistently firing correctly. The root cause of the staple misalignment could not be conclusively determined.A device history record review was performed on the device with no evidence to suggest a manufacturing related cause. An investigation within teleflex was initiated to further evaluate this complaint issue.Teleflex will continue to monitor and trend on complaints of this nature.
