H10: manufacturing review: the device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: one trocar stylet was received without the hub.During visual evaluation, it appeared the needle was detached from the hub, the needle appeared to have residue throughout and no other anomalies were observed.Using a black light (under sandblasting process), it was noted no adhesive residue was on the trocar stylet where the hub would have been.Upon dimensional observation, the length of the returned stylet measured and the measured within specification. Therefore, the investigation is confirmed for the reported detachment of device as the returned trocar stylet was detached from its hub and the investigation is confirmed for the identified nonstandard device as a lack of adhesive residue noted on the trocar stylet. The cause for the reported detachment of device or device component and the identified nonstandard device was traced to be a manufacturing related issue.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.H10: d4 (expiration date: 04/2026), g3, h6 (device).H11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
