Event or Problem Description
It was reported that a patient underwent a primary breast augmentation surgery with implantation of a 475cc mentor memorygel breast implant prosthesis.Post-operatively, the patient was diagnosed with bilaterally ruptured breast implants using magnetic resonance imaging.Subsequently, a lymph node biopsy was performed and the patient was diagnosed with granulomas of the breasts. As a result, the patient is scheduled for bilateral breast implant removal and replacement on (b)(6) 2024.This report is for the left breast prosthesis.Refer to manufacturing report number 645337-2024-04265 for the contralateral event.

Additional Manufacturer Narrative
On may 16, 2024, mentor received the following additional information. It was reported that the granuloma was only present in the right axillae per ultrasound imaging and the biopsy was only performed on the right breast.Additionally, per magnetic resonance imaging, there was implant site fluid collection both breasts and a left breast mass. The patient underwent bilateral replacement with 500cc mentor memorygel breast implants during the removal surgery. As this report is for the left breast, surgical intervention and granuloma are no longer applicable.Manufacturer¿s reference number: (b)(4).

Additional Manufacturer Narrative
On june 26, 2024, mentor completed an evaluation on the returned device.Mentor conducted a visual inspection and microscopic examination of the device.Visual analysis of the returned sample revealed that the breast implant was found to have a tear at the junction between the shell and patch.A microscopic examination was performed, and the cause of the tear could not be identified.Although no conclusion could be reached on the cause of the reported event, the instructions for use contain the following caution: rupture can occur after implantation but is more likely to occur the more prolonged the implant is implanted. The following may cause the implant to rupture: stressing the implant during implantation and weakening it; folding or wrinkling of the implant shell; excessive force to the chest (e.G.During closed capsulotomy); trauma; compression during mammographic imaging; and severe capsular contracture.Breast implants may also wear out over time.Manufacturer¿s reference number: (b)(4).
