Additional information: section d9 - device available for evaluation? yes section d9 - date returned to manufacturer: 18-jul-2024 section h3 - device evaluated by manufacturer? yes device evaluation: the metal splint was not returned for evaluation; however, an image and dimensions were provided. The device showed signs of being well used and autoclaved with a couple of nicks in the center rod and some discoloration of the injector body.None of this gives cause for concern and the device remains fully operational. It has not been possible to ascertain where the metal fragment did come from, there were no visible signs that the fragment was once attached to the device. This would be evident by the appearance of an ¿uncolored¿ area on the device. Also, based on the physical size of the metal fragment, it could not have come from the inserter. Conclusion: as a result of the investigation there is no indication of a product quality deficiency and the reported issue could not be verified. All pertinent information available to johnson & johnson surgical vision, inc.Has been submitted.
