Additional Manufacturer Narrative
The event unit was returned to applied medical for evaluation.Testing was performed on the event unit, which confirmed that the grasper could not be actuated.Visual inspection was performed where the jaw post was observed to be dislodged from the grasper head.Based on the condition of the returned unit, it is likely that the reported event was caused by the jaw post dislodgment which resulted from a large actuation force applied to the handle.Applied medical has performed a historical trend analysis and review of production records and no trends were identified. The probability and criticality of harm resulting from this failure have been evaluated and were found to be at an acceptable level.

Additional Manufacturer Narrative
The event unit returned to applied medical for evaluation, along with an additional sterile non-event unit from the same lot.The complainant¿s experience of being unable to open the jaws was confirmed.Functional testing was performed where the event unit was unable to be actuated, as neither the handle nor jaws would open.Visual inspection was performed on the returned event unit, where it was observed that the jaw posts of both jaws had dislodged from the curved slot track.Visual and functional testing was performed on the additional sterile non-event unit from the same lot, where no damages or non-conformances were observed. The sterile non-event unit was able to be actuated and the locking mechanism was functional without issue.Based on the condition of the returned unit, it is possible that the damaged components resulted in the dislodgement of the jaw post. However, applied medical has reviewed the details surrounding the event and related products and is unable to determine the exact cause of the reported event based on the evaluation of the returned unit.Applied medical has performed a historical trend analysis and review of production records and no trends were identified. The probability and criticality of harm resulting from this failure have been evaluated and were found to be at an acceptable level.Correction: section h6 (investigation conclusion) is being updated to "4315 - cause not established".Section h11 contains an updated narrative based on the updated investigation.
