Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: according to the information received, "at a general check of the attune ap cutting blocks the central sterilization department detected substantial sharp edges at the corners of the cutting slots. There were no implications during any surgery! no further information available".The product was returned to depuy synthes for evaluation.Visual inspection of the returned sample attune a-p chamfer block revealed that the corners have burrs on the edges and sings of nicks were identified, most likely caused by the repetitive interaction with the saw blade. The damage observed is mostly consistent with a saw blade making contact with the device outside of its intended range/cutting section.Properly handling and attention to the approved use of the device diminishes the risk of failure. However, based on the age of the device (over 5 years old), reported condition can be traced to an end-of-life problem, where damage is consistent with repeated use and servicing.A functional inspection was not performed since it was not applicable to the complaint condition.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the attune a-p chamfer block would contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to end of life, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
