H10: manufacturing review: the device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: one opened 10g visiloc mri introducer set was returned for evaluation. On visual evaluation, visiloc introducer set was received with protective tubing and appeared to be clean. The obturator was received in two and black residue was observed in the tray.On microscopic observations, a circumferential break was noted to the obturator, partial liquid was noted to be missing within the obturator. No other anomalies were noted.Further functional testing was not performed, due to the condition of the device returned. Therefore, the investigation is confirmed for the identified break issue as a circumferential break was noted to the obturator. However, the investigation is kept as inconclusive for the reported device contamination issue as the identified break issue contributed the leak in the obturator.A definitive root cause for the alleged device contamination issue is due to the identified break issue noted in the obturator. However, the definite root cause for the identified break issue could not be determined based upon the provided information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.H10: d4 expiry date: 04/2024.H11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
