Event or Problem Description
An attempt was made to use one onyx frontier coronary drug-eluting stent to treat a lesion. The device was inspected with no issues noted.Negative prep was not performed. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was not noted while advancing the device.Excessive force was not used.It was reported that stent deformation occurred in vivo during positioning/advancement.It was detailed that when the stent reached the lesion something felt funny. It was decided to stop and retrieve the stent. It was noted that a stent/spur malformation had occurred. The stent was replaced with another same size onyx frontier stent however, a non-medtronic guide extension catheter (gec) was used to delivery the stent to the lesion. The onyx frontier stent was expanded with no issues. The patient is alive with no injury.
