New information added to the following fields: d8, d9.This report is being supplemented to provide additional information based on the results of third-party testing, the device evaluation and the lms final investigation.Despite good faith attempts the user cleaning disinfection and sterilization (cds) processes were not shared.Olympus/ the user facility provided the following result of the culture test, performed at the third-party labs: sampling date: (b)(6) 2024.Sampling from: all channels.Cfu: <1cfu.Bacterial identification: n/a.The device was evaluated where the following abnormalities were found that could have led to the reported event; the device was repaired by a third party including the bending section cover adhesive, the insertion tube, the insertion tube protector, the scope connector and the switch button. As the device was repaired by a third party with non-olympus materials; the defined biocompatible characteristics are no longer guaranteed, for example; it was unknown if an improper adhesives was used or improper materials were used.Additionally, the defined mechanical characteristics are no longer guaranteed; for example the melting of improper insulation material instead of the ceramic manufactured by olympus.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a root cause could not be determined.Growth of microorganisms were found through culture testing by the user after reprocessing. However, when olympus culture tested after reprocessing in accordance with the ifu before repair, the results conformed to the regulation's recommendation. The following is included in the device ifu: "an insufficiently reprocessed endoscope and/or accessory may pose an infection control risk to the patients and/or operators who touch them." as a result of confirming the contents of the instruction manual, reprocess method is described on the items below; chapter 6 compatible reprocessing methods and chemical agents chapter 7 cleaning, disinfection, and sterilization procedures olympus will continue to monitor field performance for this device.
