Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: it was reported that on (b)(6) 2024, the red knob broke off the femoral introducer. The black tib handle was slightly bent and would not connect, and tibia tower was binding the drill. The sigma impaction handle stuck. The product was returned to depuy synthes for evaluation.Visual inspection revealed signs of usage on the overall device surface, and both device's body and plunger disassembled.Additionally, the following components were not returned for evaluation: the spring, the locking pin, and the cross pin of device.Since there is no certainty why not all the main components were returned for evaluation, a potential cause was not established.A dimensional inspection was not performed since it was not applicable to the complaint condition.A functional test was not performed since the main components were not returned. The overall complaint was not confirmed as the observed condition of the hp universal handle would have not contributed to the complained issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is required. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code a0609 provided is not a valid finished goods lot number.H11 additional narrative: corrected: h3.
