Block b3: approximated based on the date the manufacturer became aware of the event.Block h6: imdrf device code a0401 captures the reportable event of pull wire broken.Block h11: the returned endovive safety peg kit pull method was analyzed. One pull method peg tube with the retrieval loop and one pull wire was returned for analysis.Visual inspection of the end of the loop confirmed the presence of damage in the form of a break.Visual inspection of the pull wire noted the presence of a tight knot/loop which was most likely the portion of the retrieval loop that broke. The pull wire was returned detached. No other problems with the device were noted. With all available information, boston scientific concludes the reported event of pull wire break was confirmed during the product investigation.Based on the condition of the returned device, it is possible that excessive force/tension applied during use caused the retrieval loop and pull wire to strain and subsequently break/detach.Therefore, the most probable root cause is adverse event related to procedure.
