Event or Problem Description
It was reported that when doing inspection for the patient, no image was found using the halogen light source.The unspecified therapeutic procedure was completed by using a similar device.There were no reports of patient harm.

The customer reports the procedure was a uterine cavity inspection and that the issue was found at preparation for use, without delay.

This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 3 years since the subject device was manufactured.The device was not returned to olympus for inspection, and the customer's complaint was not confirmed.Based on the results of the investigation, a definitive root cause could not be determined.Olympus will continue to monitor field performance for this device.
