H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of a broken guidewire is confirmed and was determined to be use related. One 18 ga powerglide pro was returned for evaluation. An initial visual observation showed abundant blood use residues throughout the returned powerglide pro.The guidewire was observed to be sticking out of the distal end of the needle. The guidewire appeared uncoiled with a break in the core wire.A microscopic observation revealed the break in the core wire had a granular fracture surface. Both the distal and proximal fracture site of the core wire was observed to have slight bends in the wire, likely from being pulled against the needle bevel. The characteristics of the break site are consistent with pulling the guidewire against the needle bevel during the insertion process. The complaint of a break in the guidewire is confirmed. This complaint will be recorded for future trending and monitoring purposes.H3 other text : evaluation findings in section h:11.
