Additional Manufacturer Narrative
Manufacturer¿s ref.No: (b)(4).The purpose of this mdr submission is to report the investigational finding of the returned device. The complaint product was returned and received for evaluation and analysis. The investigation is documented below.Investigation summary: a non-sterile 4mm x 8cm galaxy g3 xsft was received contained in the decontamination pouch.Visual inspection was performed. The introducer and the core wire were observed kinked. The coil was noted inside of the introducer component. The coil component was inspected under the microscope and it was observed to be in good condition. It was still attached to the resistance heating (rh) coil, which had not been softened indicating that the resistance heating coil had not been heated and the detachment process was not initiated. The issue documented that the introducer sheath zipper broken was confirmed since the damages noted in the introducer and core wire components prevented the device from being properly resheathed. With the limited information available, the root cause of such damages cannot be conclusively determined.According to the risk documentation, friction is a potential failure mode that can occur during microcoil re-sheathing; friction can cause force to be inadvertently applied, resulting in the damages observed. There could be other factors that may have contributed to the issue encountered during the procedure. There is no indication that the issue reported in the complaint results from a defect inherently related to the device. The issue reported that the complaint device was stretched was not confirmed since such condition was not observed on the returned device.A manufacturing record evaluation was performed for the finished device 30778048 number, and no non-conformances related to the malfunction were identified. As part of the cerenovus quality process, all devices are manufactured, inspected, and released to approved specifications. It should be noted that multiple factors could cause product failure. The instructions for use (ifu) does contain the following recommendation: do not fasten the rhv valve too tightly around the introducer sheath since excessive pressure may cause damage to the introducer sheath and/or the microcoil as it is advanced into the infusion microcatheter.Additionally, if the introducer tip and microcatheter hub are misaligned, damage may occur to the microcoil as it passes through this transition.Based on the manufacturing documentation review, there is no indication that the event is related to the device manufacturing process. As part of the post market surveillance program, information from this complaint is trended for statistical signals and corrective / preventive action may be triggered at a later time.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.Updated sections: b.4, g.3, g.6.H.2, h.3, h.6, and h.11.The manufacturer will submit a supplemental report if new facts arise which materially alter information submitted in a previous mdr report.Additional information will be submitted within 30 days of receipt.
