Event or Problem Description
The reporter indicated that the surgeon implanted a 12.1mm vticm5_12.1; -13.5/1.5/090 (sphere/cylinder/axis) implantable collamer lens into the patient's right eye (od) on (b)(6) 2024.The lens was implanted upside down.Intraoperatively the lens was removed and replaced with a same length lens and the problem was resolved.There was no patient injury.
