Additional Manufacturer Narrative
|
|
This report is being sent as follow-up no.1 to provide additional information in section and to provide the completed investigation results. The actual device is not available for return; therefore, an evaluation of the actual device was unable to be conducted. The complaint cannot be confirmed for sheath separation because a sample was not returned for assessment.Without a sample, the exact root cause cannot be determined.Field clinical was informed about the sheath separation and said that it is unlikely that the sheath would reach the ivc from the femoral access site as the ik (introducer kit) sheath is short. It is possible a spasm occurred at the femoral access site that caused the sheath to separate during withdrawal. In the absence of details regarding the size of the artery and the access site, no conclusions can be made regarding the cause of the alleged sheath separation.Review of dhr (device history record) showed there were no issues noted during manufacture of the product that could have contributed to this complaint condition.Currently no action is recommended since this risk evaluation is within the predetermined limits in the hbrt (hazard based risk table).
