Additional Manufacturer Narrative
Investigation evaluation: the product said to be involved was returned in an open pouch from the lot number provided in the report. The label matches the product returned. Our evaluation of the product said to be involved confirmed the report. The coil spring has uncoiled and both the flat and safety wires have broken, both segments remain attached by the uncoiled core wire. The core wire began uncoiling 78.5cm from the distal tip of the wire guide. The uncoiled segment stretched from 78.5cm to 186.3cm from the distal tip.A bend was noted 206.0cm to 220.8cm, 263.4cm, and 382.6cm from the distal tip.The total length of the wire guide was has extended to 529.4cm due to the stretched coil spring and broken flat and safety wires.A product discrepancy or anomaly that could have contributed to this reported occurrence was not observed. The device history record for the lot number said to be involved was reviewed.A discrepancy or anomaly was not observed with the product that was released for distribution.Investigation conclusion: our evaluation of the product said to be involved confirmed the report.A definitive cause for this observation could not be determined because the actual use conditions could not be duplicated in the laboratory setting.Due to a variety of clinical conditions such as patient anatomy, endoscope position or progression of disease state, we could not reproduce the actual conditions of product usage during our laboratory analysis. This limits our ability to conclusively determine a cause. In the report it states the wire guide was left in place during current application, the ifu states: "remove this wire guide before applying electrical current." prior to distribution, all standard wire guides are subjected to a visual inspection and functional testing to ensure device integrity.A review of the device history record confirmed that the lot said to be involved met all manufacturing requirements prior to shipment.Corrective action: a review of the complaint history was conducted and this represents an isolated occurrence. The likelihood of occurrence is considered rare.Corrective action is not warranted at this time based on the quality engineering risk assessment.Quality assurance will continue to monitor for complaint trends and reassess the risk assessment results as post market feedback continues to become available.Additional comments: based on the information provided that the wire guide was left in place during current application, a cook representative has been directed to contact the medical facility involved in an effort to promote further education and understanding related to appropriate usage of this product.
