Event or Problem Description
It was reported that during a labrum refixation surgery with knotless suturetak when inserting the anchor, the peekhart body held under tension, but when shuttling and inserting the tension and main suture, the anchors came loose and were discarded and replaced by 2 new anchors.According to the surgeon no harm for patient, operator or third party occurred.The surgery was finished successfully with a new device with the same part number.It was not necessary to switch the surgical technique or do a second surgery.
