Event or Problem Description
The reporter indicated that the surgeon implanted a 12.6mm vicm5_12.6; -10.00 diopter implantable collamer lens into the patient's right eye (od) on (b)(6) 2024.The lens tore during injection/deliver into the eye.Intraoperatively the lens was removed and replaced with a same length lens and the problem was resolved.There was no patient injury.The cause of the event was reported as unknown.
