Additional Manufacturer Narrative
H3, h6: the device was not returned for evaluation; therefore, a device analysis could not be performed. However, the photographs were reviewed and revealed that the device shows significant signs of wear. The surface of the device shows scratches, dents and gouges. The clinical/medical investigation concluded that, although the provided undated, unlabeled x-ray photo confirms the insert was disengaged confirming the report, with the limited information provided, the definitive clinical root cause of the reported adverse event could not be determined.Without the implantation date and the patient¿s immediate post implantation imaging for comparison, we are unable to determine whether the insert selection was appropriate, if the insert was locked and aligned during the primary surgery, strenuous activity/ trauma contributed to reported migration or if the reported 1 cm migration occurred over time.It is unknown if the instructions for use document for knee systems was adhered to.Therefore, it cannot be concluded that the reported adverse event is related to a mal performance of the implant or implant failure. The patient impact was the revision.Device batch number was not provided, thus, an evaluation of the manufacturing records could not be performed.A review of complaint history for the previous 12 months did not reveal similar events for the listed device.A review of the instructions for use documents for knee systems revealed that improper fixation, and/or migration of the components could cause possible adverse effects. Also, the warnings and precautions section establishes that the implant can become damaged as a result of strenuous activity or trauma. The patient should be warned of surgical risks, and made aware of possible adverse effects.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include abnormal loading of limb, alignment, insert selection, patient anatomy, procedural/user error, strenuous activity and/or traumatic injury.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.H10- additional information d6b- if explanted, give date h11- corrected data, b5- describe event or problem, d8- device available for evaluation? h6- health effect - clinical code.
