Additional Manufacturer Narrative
Block h6: imdrf device code a0501 captures the reportable event of tip detachment of device or device component.Imdrf patient code e2008 captures the reportable event of unretrieved device fragments.Block h11: a wallflex esophageal stent was received for analysis.Visual examination of the returned device found the stent fully covered and undeployed.Functional inspection was performed by actuating the delivery system (slide the handle back along the stainless-steel tube) and no problems were noted with the deployment of the stent. No other problems were noted with the stent and delivery system.Product analysis did not confirm the reported event of tip detachment of device or device component as the tip of the device was returned in good condition. There is no indication of what the customer reported because the stent was returned fully covered and undeployed with its tip in good condition. Therefore, a review and analysis of all available information indicated the most probable cause is no problem detected.A labeling review was performed and, from the information available, this device was used in a manner inconsistent with the ifu (instructions for use) / product label. The complainant reported that the axios stent was used to treat a gastrectomy leak. The ifu states, "the stent is indicated for maintaining esophageal luminal patency in esophageal strictures caused by intrinsic and/or extrinsic malignant tumors, occlusion of concurrent esophageal fistulas and treating refractory benign esophageal strictures.".
