H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of difficulty infusing into the purple lumen of a catheter is confirmed and was determined to be related manufacturing related. One 5 fr d/l powerpicc solo was returned for evaluation.A functional test of attempting to flush each lumen of the returned catheter with water using a 12 ml syringe revealed the red lumen to be patent to infusion. The purple lumen was observed to have resistance during flushing.A guidewire was subsequently used to identify a potential occlusion in the purple lumen side of the catheter. The purple lumen was found to be partially occluded at the molded suture wings. The catheter was then cut at the potential occlusion location of the molded suture wings to observe the partial occlusion.A microscopic observation revealed excessive clear over molding material inside the purple lumen of the catheter.Because the catheter was found to be partially occluded with over molding material, the complaint of difficult infusion of the catheter is confirmed and was determined to be related to manufacturing. This complaint will be recorded for future trending and monitoring purposes.
