Event or Problem Description
|
|
It was reported that the right ventricular (rv) lead triggered alerts due to high pacing impedance, high defibrillation impedance, and high superior vena cava (svc)coil impedance. It was noted that these measurements "are all experiencing spikes".The pocket was opened and it was found that there was a loose connection between the implantable cardioverter defibrillator (icd) svc port and the rv lead, as the svc coil had not been fully inserted into the header, and the set screw had not been fully deployed. The lead was reinserted into the device. The icd and lead remain in use.No patient complications have been reported as a result of this event.
