On 05/16/2024, it was reported by a sales representative via (b)(4) that an 0826-000 impactor pad de-threaded the metal coupling where it attaches to the 125° jig.As a result, the impactor pad could not be threaded into the 130° jig. While using the 130° jig, the sleeves' retaining button was sticking, preventing them from holding in place. This was discovered during a case with no patient harm.
